Terms for using Chartr.
These terms describe acceptable use of Chartr while the product supports clinical documentation workflows.
Clinical review required
Chartr is not a medical decision-maker. Clinicians must review, edit, and approve every transcript, generated note, assessment, outcome, reminder, discharge summary, and exported record before relying on it.
Authorized use
Use Chartr only for patients and clinical information you are authorized to document. Do not use Chartr in emergencies or as a substitute for direct patient care, professional judgment, regulatory duties, consent requirements, or clinic policy.
Accounts and security
You are responsible for maintaining access to your account, protecting your device, using appropriate device security, and reporting suspected unauthorized access to Chartr support.
AI-generated content
AI output may be incomplete, inaccurate, or formatted differently than expected. Chartr may omit unsupported details and may mark generated notes as needing review. Final clinical documentation remains the clinician's responsibility.
Patient data and consent
You are responsible for obtaining any required patient consent before recording, transcribing, generating notes, sending reminders, or storing patient information in Chartr.
Service availability
Chartr depends on cloud services, network connectivity, Supabase, OpenAI, and the Chartr API. Features may be unavailable during maintenance, outages, configuration changes, or third-party service interruptions.
