Chartr App Guide
How Chartr turns visits into reviewed notes.
This guide explains the clinician workflow inside Chartr: sign in, manage patients, record visits, generate draft notes, review documentation, and keep clinical work organized.
Chartr is a documentation assistant, not a diagnostic tool, treatment engine, medical device, or replacement for professional judgment. Clinicians must review and approve generated content before using it in a chart.
Getting Started
Sign in and set up your workspace.
New users can start with Apple, Google, or email. After creating an account, Chartr guides the clinician through workspace setup.
- Add clinician and clinic profile details.
- Choose profession, language, and note template preferences.
- Use secure sign-in and optional biometric lock where available.
- Return to the app later and continue from the same workspace.
Main Navigation
Five areas keep work organized.
The tab bar separates patient care, visit recording, saved notes, clinical forms, and account controls so clinicians can move quickly between daily tasks.
Record Visits
Capture the visit, then choose how to process it.
Select a recent patient, search the patient list, add a new patient, or keep the recording unlinked.
Attach the visit to a case or appointment when applicable so notes, assessments, and outcomes stay organized.
Grant microphone permission, record the visit, then stop when the clinical conversation is complete.
If the app is online, Chartr can begin transcription. If the API or network is unavailable, the recording can be kept for retry.
AI Drafting
From audio to structured note.
The processing screen shows three steps: transcribing the appointment, generating a clinical note, and saving to the account.
- Audio is uploaded for transcription.
- The transcript is used to draft a structured clinical note.
- Unsupported or missing details should be reviewed carefully.
- The clinician edits and approves the final saved note.
Note Review
Edit, export, and add addendums.
Saved notes include transcript text and clinical note sections such as Subjective, Objective, Assessment, and Plan.
- Notes can be copied or shared for EMR workflows.
- Clinicians can edit notes during the open edit window.
- Older notes move to addendum-only or locked states.
- Deleted notes can be recovered from the app's deleted records area when eligible.
Patient Workspace
Everything starts from the patient chart.
Each patient profile brings the clinician's core work into one place.
- Patient details and primary complaint.
- Active cases and visit count.
- Saved notes and full history.
- Assessments, outcomes, follow-ups, and appointments.
Cases
Group visits by episode of care.
Cases help organize notes, assessments, outcomes, follow-ups, and discharge workflows around a specific complaint or treatment episode.
Forms
Assessments and outcomes live beside notes.
The Forms workspace supports structured clinical assessments, outcome measures, saved forms, and in-progress work.
- Resume incomplete assessments.
- Start specialty assessment templates.
- Launch outcome measures.
- Attach forms to patients and cases.
Settings
Control profile, privacy, and support.
Settings includes profile editing, appearance, biometric lock, notification preferences, privacy controls, deleted records, audit log, legal links, support, and account deletion.
- Use biometric lock where supported.
- Open privacy, terms, and help pages from inside the app.
- Request account deletion from the app or support.
Privacy And Safety
Built for clinician-reviewed documentation.
Chartr processes clinician account data, patient records entered by clinicians, visit audio, transcripts, generated notes, assessments, outcome measures, follow-ups, audit events, and support metadata.
- Clinicians should confirm consent before recording a visit.
- AI-generated notes must be reviewed before use.
- Secure sign-in and optional biometric lock help protect access.
- Account deletion can be requested from settings or support.
